You can get information about item at the specified coordinates:

var RVData: TCustomRVFormattedData;

    ItemNo, Offs: Integer;

  inc(X, rv.HScrollPos);

  inc(Y, rv.VScrollPos*rv.VSmallStep);

  rv.RVData.GetItemAtEx(X,Y,RVData,ItemNo,Offs);

  if ItemNo<0 then

    exit;

  // item is the ItemNo-th item in RVData.

Where (X,Y) - client coordinates, for example coordinates passed in

OnRVMouseUp.
